100 mg twice daily for 5-7 days (Macrobid)
5-7 mg/kg/day in 4 divided doses (Macrodantin)
Avoid if CrCl < 30 mL/min; Beers Criteria drug
400 mg/day (Macrodantin)
Contraindicated if CrCl < 30 mL/min
Contraindicated in patients with previous nitrofurantoin-associated hepatic toxicity
Nausea, headache, flatulence, vomiting, dark urine
Pulmonary toxicity, hepatotoxicity, peripheral neuropathy, hemolytic anemia
Anuria, oliguria, significant renal impairment, pregnancy at term

Magnesium antacids, probenecid
Take with food to improve absorption and tolerability

Contraindicated at term (38-42 weeks) due to risk of neonatal hemolytic anemia
